A: We offer our HOSPITAL Trace report on a "No Find - No Fee" basis.
Our PHARMACY Trace report is not offered on this basis. Our
COMPREHENSIVE Trace report (which is a combined report,
consisting of the previous two) is offered on a partial "No Find - No Fee"
basis, meaning if no hospital records are found, the fee will be reduced.
Q: How do we receive the finished report?
A: You can choose to receive it either via the Internet (i.e., on a secure web
page) or by fax.
Q: Please explain the ordering process.
A: You can order either via the Internet or by fax. If you order via the Internet,
simply use our Online Order Form. In this case
it is necessary to also fax us your signed medical authorization.
Alternatively, you can use our Fax Order Form to handle the
ordering process completely by fax.
Q: How can we obtain copies of any medical treatment records found?
A: Ordering information and cost (when available) is provided in your report.
