The PHARMACY TRACE Report consists of two main parts:

1. Identity verification and address history

2. a search of pharmacies corresponding to your subject's address history (or to geographic areas you specify).

Required from Client: DOB and/or SSN of subject

Your PHARMACY Trace Report will be posted to a secure web page and you'll receive the password by email, usually within 3-4 business days. (Delivery by fax also available.) (View Sample)

Your PHARMACY Trace Report will contain the following specific information:

• SSN Trace. Using our proprietary databases, we will verify your subject's identity, based on his/her SSN, for use in developing your report.
  
  
• Address History. A list of your subject's addresses for the past seven years.
  
  
• Aliases. Your subject's SSN will possibly reveal aliases (alternative names) he/she has been using, including (in the case of a female subject) her maiden name. To ensure thoroughness, E-MedicalTrace will also research aliases in preparing your report.
  
  
• Age. Your subject's age or month/year of birth, if available
  
  
• Pharmacies and Prescribed Medications. Names/addresses of all pharmacies found in areas corresponding to your subject's address history. All those which have dispensed medications to your subject will be indicated and all other available information supplied, e.g., medications dispensed, dates dispensed, etc.
