The HOSPITAL TRACE Report consists of two main parts:

1. Identity Verification and address history, and

2. a search of hospitals in areas corresponding to your subject's address history (or to areas you specify).

Required from client: Subject's DOB and/or SSN; signed medical authorization.

Your HOSPITAL Trace Report will be posted to a secure web page and you'll receive the password by email, usually within 3-4 business days. (Delivery by fax also available.) (View Sample)

Note: There is no charge for the Hospital Trace Report if no medical treatments are found

Your report will contain the following specific information:

  • SSN Trace. Using our proprietary databases, we will verify your subject's identity, based on his/her SSN, for use in developing your report.

  • Address History. A list of your subject's addresses for the past seven years.

  • Aliases. Your subject's SSN will possibly reveal aliases (alternative names) he/she has been using, including (in the case of a female subject) her maiden name. To ensure thoroughness, E-MedicalTrace will also research aliases in preparing your report.

  • Age. Your subject's age or month/year of birth, if available

  • Medical Facilities and Treatments. Names/addresses of all hospitals found in areas corresponding to subject's address history. All available records and treatments found are detailed for each facility, including all supplied information, such as treatment type (ER, Same Day Surgery, Xray, Outpatient, Inpatient, etc.) and dates of treatment.
